Social Care Project Manager

Transforming Care Services are currently working in partnership with a social care provider who is known for providing high quality support services to adults with learning disabilities, mental health issues, physical disabilities, complex needs, dementia and older people. They have services across the UK and provide residential, supported living and extra care services for adults. Do you have the following experience in developing and coordinating a flexible and high quality service for people with Mental Health needs. Have supervised and managed a team of staff, providing person centred support to enable people with mental health needs to live independently in the community, build daily living skills, improve their wellbeing and maintain their health.Knowledge of the range of community care and housing services.Demonstrable understanding of mental health needs (including signs and symptoms, treatment approaches and interventions) This is a unique project that comprises of 25 independent flats, each individual will live in this accommodation for a maximum of 2 years, the project manager will be responsible for multi-agency working to ensure consistency and a coordinated service support is provided to all. To ensure that the service is provided with the service specification and the service contract. And to comply with all monitoring and evaluation requirements. At least one years experience of supervising staff or managing services for people. Experience of supporting people including individuals with mental health needs. Experience of multi-agency working, experience of networking and liaising with a wide range of agencies.Experience of organising and prioritising own work and experience of evaluating, monitoring and reviewing services. You must have possession of Dip SW, RMN, NVQ IV, RMA or equivalent professional Qualification or working towards this, and driving licence and use of a car.
