We now provide adult social care services alongside our established healthcare services, which has given us the chance to develop better, more joined up ways of working that deliver improved outcomes for the local community.
Salford´s Intermediate care service sits under the newly formed Integrated Care Organisation.
Integrated care is all about providing better care and support to our patients, users and the people of Salford by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.
Salford Together is a partnership between NHS Salford Clinical Commissioning Group,Salford City Council, Salford Royal and Greater Manchester West Mental Health NHS Foundation Trust. Together, this partnership is working to integrate health and social care for older people in Salford, bringing the contributions of GPs, District Nurses, social workers, mental health professionals, care homes, voluntary organisations and local hospitals into a more aligned system and provide older people with the support they need to manage their own care.
We currently have 3 posts available
Permanent Post - Limited rotation across Intermediate care including
community rehab/falls, supported discharge, rapid response and bedded rehab unit.
2 Temporary posts until March 2019 - Based with in the community rehab and falls team to support the development of a comprehensive falls management pathway across the community.
You will be expected to work as part of the team using the Single Assessment Process and deliver Physiotherapy interventions to patients in their own home including within residential and nursing homes.
You will be a registered Physiotherapist with current HCPC registration and have some pre or post registration experience within the community and/or intermediate care experience. You will be able to deliver a high standard of evidence based individual care, have excellent interpersonal skills to
enable you to work effectively as part of a multi-disciplinary team, and be flexible to meet the needs of a 7 day service.
