Wakefield Council Location:
We are looking to appoint a 26.25 hours self-motivated and enthusiastic Reablement assistant to work within Wakefield Council´s Integrated Care Service working closely with NHS Community Health Services. This is a temporary position to cover a secondment for approximately 8 months.
The Reablement service has teams located in the 3 locality hubs across the district and also works closely with the Hospital Social Work Team. This post will be based within the Waterton hub located in Wakefield.
This is an essential service which works 7 days per week 52 weeks of the year and involves covering a 2 week rota and includes early shifts from 06.30am afternoon shifts to 23.00pm, weekends and bank holidays. Applicants must be car drivers and have the use of a vehicle.
Reablement is a short and intensive service, usually delivered in the service user´s own home, which is offered to people with disabilities and those who are recovering from an illness or injury.
The purpose of Reablement is to help people who have experienced deterioration in their health and, or have increased support needs to relearn the skills required to keep them safe and independent at home

The successful applicant will be required to complete a Disclosure Application Form and to provide criminal conviction information.
